MOTOR VEHICLE RECORD CHECKS

(California & Federal Law / DOT Commercial Drivers)

DISCLOSURE

R&B Wholesale Distributors, LLC. ("Company") may obtain information about you from the California Department of Motor Vehicles ("DMV") and/or a consumer reporting agency for employment, continued employment, or contracting purposes.

The information obtained may include, but is not limited to:

• Driver's license status
• Motor vehicle violations
• Accident history
• Points, suspensions, or revocations
• Compliance with state and federal driving requirements

This report may constitute a consumer report and/or an investigative consumer report under the Fair Credit Reporting Act (FCRA) and the California Investigative Consumer Reporting Agencies Act (ICRAA).

ONGOING & ANNUAL DMV CHECKS DISCLOSURE

I understand that if driving is a requirement of my position, the Company may obtain motor vehicle records on an ongoing basis, including annual DMV checks, throughout my employment or engagement, to the extent permitted by law.

These checks may be conducted:

• Annually
• Periodically
• Following an accident or reported driving incident

DOT / COMMERCIAL DRIVER DISCLOSURE

(Applicable to CDL or DOT-regulated positions)

If I am applying for or employed in a position subject to U.S. Department of Transportation (DOT) regulations, I understand that the Company may obtain and review my motor vehicle record as required by applicable federal and state law, including 49 CFR Part 391.

This may include:

• Initial pre-employment motor vehicle record checks
• Annual motor vehicle record reviews
• Additional checks as required by DOT or Company policy

I understand that failure to maintain a valid commercial driver's license (CDL), medical certification, or acceptable driving record may result in disciplinary action, up to and including disqualification from driving duties or termination, as permitted by law.

CALIFORNIA DISCLOSURE

Under California law, I have the right to request information regarding the nature and scope of any investigative consumer report obtained about me.

• The Summary of Rights under the Fair Credit Reporting Act, and
• The California Summary of Rights under the ICRAA.

AUTHORIZATION

I hereby authorize R&B Wholesale Distributors, LLC. to obtain my motor vehicle driving record from the California Department of Motor Vehicles and/or a consumer reporting agency for lawful purposes related to my employment or engagement.

This authorization includes consent for:

• An initial motor vehicle record check
• Annual and ongoing DMV checks
• DOT-required motor vehicle record reviews (if applicable)

This authorization remains valid throughout my employment or engagement unless revoked by me in writing.
