We are seeking a dedicated Physical Therapist to join our team at Quinn Meadows Rehab & Care, located in Pocatello. In this role, you will play a vital part in enhancing the quality of life for our residents by developing and implementing personalized treatment plans that align with our mission of providing exceptional care.

Responsibilities
- Evaluate patients' physical abilities and limitations to develop appropriate treatment plans.
- Implement therapeutic interventions to improve patients' mobility and overall function.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Educate patients and their families on treatment plans and home exercises.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain accurate documentation of patient assessments and treatments in EMR systems.
- Participate in discharge planning to ensure continuity of care.
- Stay updated on best practices and advancements in physiotherapy.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Valid state licensure as a Physical Therapist.
- Strong knowledge of anatomy, physiology, and medical terminology.
- Experience with acute care settings preferred.
- Familiarity with EMR systems is a plus.
- Excellent communication skills for patient interaction and teamwork.
- Ability to develop effective care plans tailored to individual needs.
- Experience in outpatient or home health settings is beneficial.
