Requirements
- Active SD or Compact RN license
- Minimum 1 year of recent LTC / SNF experience
- BLS (AHA) required
- Strong assessment, communication, and time-management skills
- Ability to work independently while collaborating with a multidisciplinary care team
- Experience working with geriatric populations preferred
Duties
- Provide daily bedside nursing care to residents in an LTC/SNF environment
- Oversee medication administration, treatments, wound care, and physician-ordered therapies
- Perform and document complete assessments, monitoring changes in condition and escalating concerns appropriately
- Develop and update individualized care plans and support care conferences
- Coordinate care with CNAs, physicians, therapy staff, and social services to ensure continuity of care
- Educate residents and families regarding medical conditions, medications, and ongoing support needs
- Assist with transfers, fall prevention, mobility support, and ADL assistance when needed
- Maintain regulatory compliance, infection control guidelines, and safety standards
- Support residents with cognitive decline, memory care needs, or behavioral changes with patience and compassion
