The RN Case Manager plays a vital role in hospital settings by overseeing the management of a typical caseload of 21-40 patients. This position involves collaborating closely with interdisciplinary teams to deliver coordinated care, ensure efficient discharge planning, and advocate for patients' needs while adhering to established clinical protocols under supervision.
13 Week Contract with the option to extend
Hours: (07:30 - 16:00)
Charting: Epic - Required
Weekends Required: Yes - every other weekend
Responsibilities
- Conduct thorough patient assessments to inform care planning
- Develop and coordinate individualized care plans in collaboration with healthcare teams
- Coordinate patient discharge processes to ensure smooth transitions
- Advocate for patient needs and preferences throughout care delivery
- Manage accurate and timely medical record documentation
- Perform utilization review to optimize resource use
- Facilitate care coordination among interdisciplinary team members
- Provide patient education to promote understanding and compliance
Qualifications
- 1 years nursing experience
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
- Registered Nurse (RN) license
- Basic Life Support (BLS) certification
- Skills in case management, patient assessment, and care coordination
- Proficiency in clinical documentation and utilization review
- Strong interdisciplinary communication and time management abilities
