Job Description:
We are seeking an experienced Registered Nurse Case Manager for a 13-week contract at a leading acute care hospital in Santa Rosa, CA. This role focuses on care coordination, utilization review, and discharge planning for adult and geriatric patients.
Job Title: RN Case Manager
Shift: Day (5x8) | 08:00 AM – 04:30 PM
Contract Length: 13 Weeks with the option to extend
Responsibilities:
- Perform care coordination and discharge planning
- Conduct admission reviews using InterQual criteria
- Assess patient needs and develop care plans
- Manage benefits eligibility and prior authorizations
- Coordinate DME (Durable Medical Equipment)
- Work with HMOs, IPAs, and interdisciplinary teams
Requirements:
- Active California RN License (pending accepted)
- Minimum 2 years acute care inpatient Case Manager experience
- Recent experience required (within last 6 months)
- Experience with Epic charting system
- Strong knowledge of InterQual criteria
- Travel experience required (seasoned traveler preferred)
Schedule & Details:
- Patient Ratio: 1:20 (up to 20–22 patients)
- Weekend Requirement: Every other weekend (at least 2 shifts per 4 weeks)
- First-time travelers NOT accepted
- Maximum 7 days RTO allowed

- Dress Code: Business casual or dark scrubs with lab coat
