RN Case Manager – Santa Rosa, CA
Full‐Time | On‐Site | Hospital Case Management
Overview
We are seeking an experienced RN Case Manager to join our fast‐paced acute care team in Santa Rosa, CA. The ideal candidate brings strong clinical judgment, excellent communication skills, and the ability to coordinate complex discharges with minimal supervision. This role requires a high level of independence, critical thinking, and the ability to collaborate effectively with patients, families, and multidisciplinary teams.
This is a 13 week contract with the opportunity to extend or go permanent within the facility.
Key Responsibilities
- Perform comprehensive care coordination and discharge planning in a high‐volume acute hospital environment
- Independently arrange DME, SNF placement, home health, and ambulance transport
- Participate in and help lead family meetings, ensuring clear communication and alignment of care goals
- Collaborate with physicians, nursing staff, social workers, and ancillary departments to support safe and timely transitions of care
- Participate in the development and ongoing review of individualized treatment plans
- Work effectively with HMOs, IPAs, and managed care partners, ensuring authorization and payer requirements are met
- Identify barriers to discharge and proactively resolve issues to support patient flow
- Document all case management activities accurately and in compliance with regulatory and hospital standards
Required Qualifications
- Active Registered Nurse (RN) license in the state of California no compact licenses from other states will be accepted
- 2 years of acute hospital Case Manager experience
- Demonstrated experience in care coordination and discharge planning in a fast‐paced setting
- Strong working knowledge of HMOs, IPAs, and managed care processes
- Proven ability to independently arrange DME, SNF, and ambulance transport
- Excellent communication, organization, and critical‐thinking skills
- Ability to lead and participate in family meetings and interdisciplinary discussions
Preferred Qualifications
- Experience with electronic case management systems
- Prior work in a large acute care hospital or trauma center
- Strong understanding of community resources and post‐acute care networks
