At Quality Care Home Health & Hospice, our Social Workers play a vital role in supporting patients and families through one of life’s most meaningful and challenging transitions. This position focuses on compassionate, patient-centered care while working closely with an interdisciplinary team to address the emotional, social, cultural, and practical needs of patients facing terminal illness.
What You’ll Do
- Conduct comprehensive psychosocial assessments of patients and families, identifying emotional, environmental, and support needs related to terminal illness
- Provide counseling, emotional support, and crisis intervention to patients and caregivers as needed
- Collaborate with nurses, physicians, and the interdisciplinary team to develop and support individualized plans of care
- Educate patients and families on advance directives and assist with end-of-life planning, including legal, financial, and funeral arrangements
- Serve as a liaison between patients, families, and community resources to address practical and environmental needs
- Support culturally sensitive care by identifying and educating the care team on patient and family values, traditions, and communication preferences
- Maintain timely, accurate clinical documentation in accordance with regulatory and organizational standards
- Participate in interdisciplinary team meetings and quality improvement initiatives
- Build and maintain relationships with community agencies and external resources to support patient care
What We’re Looking For
Education:
- Master’s degree in Social Work (MSW) from a CSWE-accredited program or
- Bachelor’s degree in Social Work from a CSWE-accredited program meeting hospice personnel requirements
Experience:
- At least one year of healthcare experience; hospice experience strongly preferred
- Understanding of hospice philosophy and end-of-life care
- Strong communication, organization, and documentation skills
- Ability to work collaboratively in a team-based care environment
- Current CPR certification
- Valid driver’s license with reliable, insured transportation
