Responsibilities
- Clinical Supervisor / RN Duties
- Oversee home care services to ensure quality, safety, and compliance
- Develop and manage care plans for clients
- Perform client assessments and reassessments
- Ensure proper clinical documentation and compliance
- Care Coordination
- Coordinate care with physicians, therapists, and families
- Manage hospital-to-home transitions and referrals
- Ensure continuity of care and positive client outcomes
- Team Leadership
- Supervise and support caregivers and healthcare staff
- Assist with hiring, onboarding, and training
- Assign staff based on client needs and skill level
- Provide coaching and performance management
- Quality Assurance
- Ensure compliance with Colorado home care regulations
- Maintain readiness for audits and surveys
- Monitor and improve service quality
- Registered Nurse (RN) OR licensed healthcare professional (as permitted by Colorado regulations)
- Minimum 2 years of healthcare experience, including: at least 1 year in home care or a closely related field
- Experience as a Nurse Supervisor, Clinical Manager, or Care Coordinator preferred
- Current licensure in good standing
- Strong clinical judgment and organizational skills
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively
Salary : $85,000 - $125,000
