This full-time role focuses on delivering direct outpatient Substance Use Disorder (SUD) services to individuals with serious mental illness and co-occurring substance use disorders in community-based settings. Responsibilities
- Provide community-based substance use treatment services to ACTT clients in homes and local community settings.
- Conduct individual, group, and family therapy sessions.
- Complete LOCAs, pre-authorizations, and clinical reviews in a timely manner.
- Develop and maintain treatment plans, NCTOPPs, and discharge/aftercare plans aligned with client goals.
- Maintain ongoing communication with families, referral sources, and support systems.
- Deliver interventions for individuals with severe and persistent mental health conditions and substance use disorders.
- Participate in on-call and emergency service rotation, including crisis planning, after-hours assessments, and hospital diversion.
- Attend treatment team meetings and required staff trainings.
- Perform additional duties as assigned.
- LCAS(A) or CADC required.
- Ability to obtain required certification/licensure within 90 days of hire (if applicable).
- Qualified Professional (QP) status per 10A NCAC 27G.0104.
- Valid North Carolina driver’s license.
- Ability and willingness to travel locally within the Jacksonville area.
