Job Title: Travel RN Case Manager / Care Coordinator

Location: Chico, CA 95926

Schedule:

• 8-hour day shifts
• 8:00 AM – 4:30 PM
• Weekdays and weekends

Compensation:

• Base Pay: $51/hour
• Meal & Lodging Stipend: $1,121.40/week
• Weekly Gross: Up to $3,161.40/week(successful completion of scheduled hours)

Job Summary:
We are seeking an experienced Travel RN Case Manager/Care Coordinator to join a leading acute care facility in Chico, CA. The Care Coordinator is responsible for coordinating patient care throughout an episode of illness or treatment by collaborating with patients, families, physicians, multidisciplinary teams, and payors. This role focuses on ensuring high-quality, cost-effective care while promoting optimal clinical outcomes and efficient resource utilization.

Key Responsibilities:

• Coordinate and manage patient care from admission through discharge or transfer.
• Assess, plan, implement, and evaluate patient care using the nursing process.
• Collaborate with physicians, nurses, therapists, patients, families, and insurance providers to develop and coordinate individualized care plans.
• Review admissions, discharge planning, observation stays, readmissions, and utilization management to ensure appropriate level of care.
• Initiate and monitor clinical pathways based on admission diagnoses.
• Advocate for patients and families while facilitating communication among the healthcare team.
• Evaluate quality and utilization cases to improve patient outcomes and resource management.
• Maintain accurate documentation using EPIC EMR.
• Support patient transitions to appropriate levels of care and community resources.
• May provide case management support in the Emergency Department as assigned.

Minimum Qualifications:

• Active California Registered Nurse (RN) License.
• Minimum of 1 year of recent Case Management experience in an acute care setting.
• Current BLS/CPR Certification.
• Strong knowledge of discharge planning, utilization review, and care coordination.
• Experience with EPIC EMR preferred.
