Position Summary:
The Legal Case Manager at the Eleanor Young Outpatient Clinic (EYOC) in Albany serves as the key liaison between EYOC, the Honor Court, and community partners within the criminal justice system. This position oversees client referrals through the Honor Court program, ensures compliance with court-mandated treatment requirements, and provides ongoing support to participants as they work toward recovery and reintegration into the community. Essential Duties and Responsibilities:
- Serve as the primary point of contact between EYOC and the Albany Honor Court, including judges, defense attorneys, district attorneys, probation/parole officers, and treatment providers.
- Coordinate referrals, screening, and intake for all Honor Court participants referred to PYHIT’s treatment programs.
- Maintain accurate and confidential case files, progress notes, and compliance documentation.
- Attend all required Honor Court sessions to provide participant updates, progress summaries, and treatment recommendations.
- Monitor participant attendance, compliance with treatment plans, and progress toward individualized goals.
- Report non-compliance, absences, or significant behavioral issues to the court and Program Director in a timely manner.
- Collaborate closely with clinicians, case managers, and other PYHIT staff to ensure coordinated service delivery.
- Facilitate participant communication with court and treatment staff, promoting accountability and engagement in the recovery process.
- Prepare and submit monthly data, progress summaries, and outcome reports to the Program Director.
- Participate in interdisciplinary meetings, case conferences, and staff trainings as required.
- Maintain professional relationships with community partners, referral sources, and the broader criminal justice network.
- Uphold confidentiality, ethical standards, and all regulatory and organizational compliance requirements.
Qualifications:
- Associates degree in Human Services, Criminal Justice, Social Work, Psychology, or a related field preferred.
- Minimum of two (2) years of experience working with justice-involved or treatment populations required.
- Knowledge of substance use disorders, treatment modalities, and community-based recovery supports.
- Strong communication, documentation, and organizational skills.
- Ability to work collaboratively with multidisciplinary teams and external stakeholders.
- Proficiency with Microsoft Office Suite and electronic recordkeeping systems.
- Valid NYS Driver’s License and reliable transportation required.
Work Environment:
This position is based at the Eleanor Young Outpatient Clinic in Albany, with regular attendance at Albany Honor Court sessions and occasional travel to other PYHIT program sites or community meetings. Standard office hours apply, with some flexibility based on court scheduling or client needs.
