Position Summary
The Registered Nurse is responsible for assessing clients' needs, developing individualized care plans, and overseeing in-home services provided by caregivers. This position works closely with clients, families, and caregivers to ensure high-quality, compassionate care while supporting seniors' ability to remain safely at home.
This position is based in St. Albans, West Virginia, with some local travel required.
Responsibilities
Responsibilities include, but are not limited to:
- Complete comprehensive in-home and telephone assessments to evaluate client needs using established policies and procedures.
- Develop individualized care management and service plans.
- Monitor client services and oversee care provided by in-home caregivers.
- Review caregiver documentation and worksheets for accuracy and compliance.
- Maintain timely, accurate documentation and client records.
- Conduct new hire orientation for caregivers.
- Provide annual caregiver recertification training, including CPR and First Aid.
- Collaborate with clients, families, caregivers, and community partners to ensure quality care.
- Respond promptly and compassionately to client and family concerns.
Qualifications
- Associate's or Bachelor's degree in Nursing.
- Active Registered Nurse (RN) license in the State of West Virginia.
- Strong assessment, communication, and organizational skills.
- Demonstrated ability to respond effectively to client and family needs.
- Home care or case management experience is preferred.
