Qualifications and Requirements:
- Assessment of psychological, emotional and/or economic factors affecting patient's limitations and potential for a lack of improvement
- Collaborates with the physician and other members of the health team to develop a Plan of Care
- Master's Degree in Social Work is required
- At least 1 year of social work experience in the healthcare field preferred
- Comply with all aspects of the company's compliance and HIPPA regulations
- Must possess a valid driver's license
