- Demonstrate a working knowledge of Medicare and non-Medicare reimbursement requirements, including eligibility for service
- Ability to quickly learn new software applications
- Experience with Availity, CarePort, etc.

Responsibilities
- The person in this role will work from our Naperville office Monday - Friday from 8:30am to 5:00pm.
- Looking for someone who is experienced and proficient, independent, and ready to excel in your role as an Intake Coordinator.
- The Intake Coordinator is responsible for the admission process for new patients by ensuring availability of reimbursement for services, identifying the clinical team to deliver care, and scheduling all patients' visits within prescribed discipline and visit frequency requirements:
- Receives referrals from Clinical Liaisons and initiates the intake process
- Enters new patient records in the system, ensuring accuracy of all patient demographic information
- Performs eligibility/insurance verification for the initial referral
- Heavy phone contact with Physician offices to ensure physician will accept home health patient for treatment; notify Clinical Liaisons of any physician concerns
- Once patient has been discharged from the hospital, coordinate referrals for service with the
- Scheduling team once proper authorization has been confirmed
- Adheres to HIPAA and patient information privacy requirements
- Perform other administrative duties as needed
Pay: $22.30 - $26.86 per hour
Expected hours: 40 per week
