Overview
We are seeking a dynamic and compassionate Client Care Coordinator to join our healthcare team. Duties

  • Act as the main liaison between patients, families, healthcare providers, and insurance companies to facilitate effective communication and coordination of care.
  • Manage patient intake processes, including gathering medical history, verifying insurance information, and scheduling appointments.
  • Assist with discharge planning by coordinating follow-up care, home health services, and necessary medical equipment.
  • Utilize electronic health record (EHR) systems to document patient interactions accurately and efficiently.
  • Conduct utilization review and management to ensure appropriate use of healthcare resources in compliance with HIPAA regulations.
  • Support case management activities by tracking patient progress, updating care plans, and collaborating with multidisciplinary teams.
  • Provide education on medical terminology, hospital procedures, and available support services to patients and their families.

Requirements

  • Proven experience in critical care settings such as ICU, emergency medicine, or hospital medicine is highly preferred.
  • Strong knowledge of medical coding systems including CPT (Current Procedural Terminology), ICD-9/10 (International Classification of Diseases), and ICD coding practices.
  • Experience working within hospital environments such as Level I or Level II trauma centers or pediatric units like PICU or neonatal intensive care units (NICU).
  • Familiarity with EMR/EHR systems and healthcare compliance standards like HIPAA is essential.
  • Excellent communication skills with the ability to handle sensitive situations empathetically while maintaining professionalism.
  • Background in nursing, social work, behavioral health, or managed care is advantageous.
  • Ability to multitask efficiently in a high-pressure setting while maintaining attention to detail.
