Job Overview/Summary
The Diabetes Patient Navigator will play a key role in supporting patients at therapy initiation and throughout their treatment journey. This role focuses on patient engagement, medication adherence support, education, and collaboration with healthcare providers to ensure a seamless and positive experience for individuals managing diabetes.
Responsibilities
- Provide personalized patient engagement from therapy initiation through ongoing treatment.
- Assist with adherence and respond to questions about medication, therapy, and patient needs.
- Deliver education, support, and training to healthcare providers and patients on on-label product administration and treatment.
- Handle initial inquiries and address basic reimbursement questions.
- Identify, gather, and document field-generated HCP insights to support innovative medical strategies while complying with corporate and industry policies.
- Demonstrate expertise in disease state, product knowledge, and the broader treatment landscape.
- Engage with target customers, including advocacy groups, in various settings such as offices and conferences.
- Provide timely follow-up to healthcare provider questions and requests.
- Educate and promote the use of client resources to optimize patient experience and care.
- Collaborate with commercial and medical counterparts within the assigned territory.
- Strong interpersonal and communication skills.
- Expertise in diabetes education and patient support.
- Ability to engage effectively with healthcare providers and patients.
- Strong understanding of reimbursement basics and patient resource navigation.
- CDCES certification highly preferred.
- Experience collaborating with cross-functional commercial and medical teams.
- Familiarity with pharmaceutical industry practices.
- Bachelor’s degree required.
- Clinical background required (LPN, RD, RN, PharmD, RPH, etc.).
- Minimum 2 years of diabetes education experience within patient support teams.
- Pharmaceutical industry experience highly preferred.

Salary : $45,000 - $50,000
