Diploma, Associate of Nursing (ASN) required, Bachelor of Nursing (BSN) preferred from an accredited school of nursing. Current Indiana Registered Nurse license and current BLS. Chemotherapy/Immunotherapy certification preferred upon hire or obtain within 6 months of hire and must be maintained. Oncology Certified Nurse (OCN) certification preferred. The nurse navigator reflects the mission, vision, and values of Pulaski Memorial Hospital, adheres to the organization’s Standards of Behavior and Corporate Compliance, and complies with all relevant policies, procedures, and guidelines. The RN Navigator is accountable and responsible for the provision and coordination of competent, evidence-based care along the care continuum which includes assessment, formulating a nursing diagnosis, outcome identification, planning, implementing and evaluating the patient plan of care. The RN Navigator communicates, collaborates and works with other members of the health team to provide safe, effective, efficient quality care. The clinical nurse participates in decision-making, teaching, and mentoring. Providing education to patients and families; coordinating patient care with supportive disciplines and in-home therapies/resources; supporting patients and providers before/during/after clinic visits; assisting with Prior Authorization Management; supporting patients to ensure ordered testing and therapies are completed per provider recommendations; administering oncology/hematology treatments and support; patient audits and data collection to support continuous Performance Improvement for the department.
