\n- Full-time Palliative Nurse Navigator
- Schedule: Monday thru Friday, 8a-5p
- Available territory openings: LPN to cover patients in 1-2 counties surrounding office
- Serve as the lead clinical navigator for palliative care patients in Hamilton and Marion counties, with potential expansion to additional service areas as the program grows.
- · Conduct initial patient and family consultations to establish goals of care and identify support needs.
- · Provide ongoing home visits, phone calls, and follow-up communication focused on education, symptom awareness, and anticipatory guidance.
- · Explain disease progression, treatment options, care pathways, and benefits of hospice when appropriate.
- · Build strong relationships that promote trust, comfort, and understanding throughout the care journey.
- · Identify early signs of decline and collaborate with the interdisciplinary team to determine hospice eligibility.
- · Coordinate warm handoffs and smooth transitions to hospice services when indicated.
- · Maintain timely and accurate EMR documentation of all encounters, assessments, education provided, and transition recommendations.

- Current LPN or RN license in the State of Indiana.
- Valid driver’s license and reliable automobile.
- Proof of current automobile insurance.
- Current health certificate/physical examination and TB testing.
- Successful completion of criminal background check.
