Responsibilities:
· Deliver physical therapy interventions as directed by the supervising Physical Therapist and outlined in the Plan of Care.
· Assist patients with exercises, mobility training, gait, balance, and other therapeutic activities to support rehabilitation and safety.
· Monitor patient response to treatment and communicate changes or concerns to the supervising PT.
· Instruct patients and caregivers in home exercises and safe use of assistive devices or techniques.
· Accurately and promptly document all services provided in the electronic medical record (EMR).
· Maintain ongoing communication with the PT to ensure coordinated, effective care.
· Ensure compliance with agency, state, and federal guidelines, including HIPAA and CMS regulations.
· Travel to patient homes/facilities within assigned geographic areas.
· Participate in interdisciplinary case conferences and supervisory support as needed.

Qualifications:
· Required: Graduate of an accredited Physical Therapist Assistant program.
· Licensure: Current and unrestricted license to practice as a PTA in the State of New York.
· Certification: Current CPR certification.
· Valid driver’s license and access to reliable transportation.
· Strong interpersonal, communication, and time management skills.
· Preferred: Experience in home health or rehabilitation settings.
Job Types: Contract, Per diem
