Job Description
We are seeking a Director of Case Management to lead and oversee case management operations across our behavioral health programs. This role is responsible for building, scaling, and optimizing a high-performing case management function that ensures patients receive seamless coordination of care across their entire treatment journey. You will work closely with physicians, therapists, nurses, and operations leadership to ensure patients are appropriately supported, followed, and transitioned across levels of care without disruption. This is a fully remote leadership role in a fast-paced clinical environment where accountability, systems thinking, and execution are critical.
Key Responsibilities
- Lead and oversee remote case management operations across multiple programs and service lines
- Develop and implement care coordination workflows across psychiatry, therapy, and outpatient services
- Ensure smooth patient transitions across levels of care and treatment programs in a virtual-first environment
- Supervise, coach, and support distributed case management staff to maintain high-quality performance
- Partner with clinical providers and operations teams to align patient care plans and follow-up strategies
- Monitor patient engagement, compliance, and continuity of care across virtual systems
- Identify breakdowns in care coordination and implement scalable process improvements
- Ensure documentation accuracy, regulatory compliance, and adherence to clinical standards
- Serve as a key liaison between clinical teams and administrative leadership
Requirements
- 5 years of experience in case management, care coordination, or clinical operations
- 2 years in a leadership or supervisory role preferred
- Experience in behavioral health, psychiatry, telehealth, or integrated care settings strongly preferred
- Strong understanding of care coordination workflows and patient navigation systems
- Proven ability to lead remote or distributed teams
- Excellent communication, leadership, and organizational skills
- Ability to operate in a fast-paced, high-accountability, virtual environment
