Responsibilities
- Follow established therapy plans, educate patients/caregivers, and document all care.
- Assist in patient progress monitoring and communicate with supervising PT.
- Manage caseload and deliver therapy services as directed.
- Participate in team meetings, training, and caseload reviews.
- Travel to patients’ homes or facilities, providing care in diverse environments.
- Support patients with therapeutic activities, transfers, and use of medical equipment.
- Adhere to safety, infection control, and agency policies while transporting equipment and supplies.
- Perform other duties and participate in special projects as assigned.
- NY State Certification as a Physical Therapist Assistant.
- Associate’s Degree in Physical Therapist Assisting from an approved program.
- Minimum two years clinical patient care experience.
- Bilingual skills may be required; basic PC skills preferred.
- Valid driver’s license may be necessary.
