Role Overview: Provides clinical leadership in an acute inpatient setting, ensuring patient/family-centered care. Demonstrates evidence-based nursing practices across assessment, planning, implementation, and evaluation, incorporating safety, quality, and cost-efficiency. Ensures compliance with organizational policies, regulatory standards (TJC, CMS), and the Nurse Practice Act. Responsible for developing and updating individualized care plans, coordinating multidisciplinary teamwork, and ensuring smooth patient transitions post-discharge.

- Perform comprehensive patient assessments and prioritize care based on data and patient needs.
- Develop, communicate, and revise individualized care plans in collaboration with patients, families, and healthcare team.
- Deliver direct patient care, delegate tasks appropriately, and monitor care quality and safety.
- Utilize performance metrics for quality improvement, document progress, and evaluate outcomes.
- Educate patients and families, addressing learning needs and promoting participation in care.
- Maintain professional licensure, complete mandatory education, and ensure documentation accuracy.
- Advocate for patients and coordinate care across the healthcare continuum.
- Associate degree in Nursing; Bachelor’s preferred.
- At least one year of RN experience or completion of a recognized residency program.
- Current RN license recognized by NCSBN Compact or applicable state.
- Strong clinical judgment, communication, and teamwork skills.
