Job Summary

We are seeking a Registered Nurse to join our Home Health team in Medford, OR. As a Registered Nurse, you will work alongside an interdisciplinary team to provide whole-person quality care to patients in need. You will be empowered by our shared governance model that includes nursing staff in the process of creating practice standards and expectations.

The ideal candidate will have 1 year of medical/surgical experience in an acute care setting and a passion for delivering high-quality patient care in a home health setting. Key Responsibilities:

• Provide comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient
• Collaborate with other healthcare professionals to ensure seamless care coordination and communication
• Administer medications and treatments as ordered by physicians
• Monitor patient progress and adjust care plans as needed
• Maintain accurate and detailed records of patient interactions and outcomes

Required Qualifications:

• Bachelor's degree in Nursing from an accredited institution
• Current Oregon Registered Nurse License
• National Provider BLS - American Heart Association within 30 days of hire
• 1 year of medical/surgical experience in an acute care setting
