Job Summary
We are seeking a Registered Nurse to join our Home Health team in Medford, OR. As a Registered Nurse, you will work alongside an interdisciplinary team to provide whole-person quality care to patients in need. Key Responsibilities:
- Provide comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication
- Administer medications and treatments as ordered by physicians
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and detailed records of patient interactions and outcomes

Required Qualifications:
- Bachelor's degree in Nursing from an accredited institution
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association within 30 days of hire
- 1 year of medical/surgical experience in an acute care setting
