Description
As a Care Navigator, you will serve as the primary point of contact for new business development, referral management, and community outreach. You will represent our hospice services to facility staff, attending physicians, hospitals, patients, and families, ensuring they understand our compassionate approach to end-of-life care. Your role will include public relations, education, and fostering relationships between our hospice team and the communities we serve.
Key Responsibilities
- Identify, develop, and maintain relationships with referral sources to drive patient admissions
- Implement and contribute to strategic marketing and outreach initiatives
- Provide education on hospice services and end-of-life care to healthcare providers, facilities, and the community
- Maintain accurate records, including sales call logs, referrals, and expense reports
- Address service concerns, resolve issues promptly, and provide exceptional customer service
- Adapt to evolving business needs and demonstrate professionalism in difficult situations
- Exhibit strong communication, negotiation, and public speaking skills
- Work independently with a self-driven, results-oriented approach
Qualifications
Education
Bachelor's degree in a relevant field preferred.
Experience
- Minimum 3 years of successful outside sales experience in healthcare, hospice, or a related industry
- Proven ability to develop and maintain referral networks
- Strong knowledge of hospice services and the ability to educate referral sources on benefits
Skills and Abilities
- Excellent verbal and written communication, including public speaking and presentations
- Ability to customize messaging based on different audiences
- Agile and creative problem-solving skills with an entrepreneurial mindset
- Self-motivated with the ability to work independently and manage a sales territory
