Position Summary

The Provider Relations Specialist – Post-Acute Care is responsible for supporting TCPA’s growth by building and maintaining strong relationships with healthcare providers, facility partners, and referral sources. This role focuses on developing new partnerships, strengthening existing relationships, and facilitating communication between referral sources and internal teams to ensure seamless patient transitions.

This is an ideal opportunity for a relationship-driven professional with experience in post-acute care, hospice, or healthcare outreach who is passionate about connecting patients to high-quality care.


Key Responsibilities

Relationship Development & Management

  • Build and maintain strong relationships with skilled nursing facilities, rehabilitation centers, hospitals, physicians, and practice administrators
  • Serve as a primary point of contact for external partners, ensuring consistent communication and responsiveness
  • Conduct regular in-person visits to facilities and provider offices to strengthen partnerships

Business Development Support

  • Identify and develop new partnership opportunities within the post-acute care market
  • Support outreach efforts through networking, community engagement, and market research
  • Assist in expanding TCPA’s presence within target markets and referral networks

Referral & Admission Coordination

  • Collaborate closely with referral and admissions teams to support timely and appropriate patient placements
  • Facilitate communication between external partners and internal clinical/operations teams
  • Help remove barriers to patient access and ensure a smooth referral process

Market Awareness

  • Maintain knowledge of local market dynamics, referral patterns, and competitor activity
  • Provide feedback to leadership on opportunities for growth and service improvement

Collaboration & Reporting

  • Work cross-functionally with clinical, operations, and leadership teams to align services with partner needs
  • Track outreach activities, referral trends, and partnership development efforts
  • Provide regular updates on relationship activity and growth opportunities
Qualifications:

Qualifications

  • Bachelor’s degree in Healthcare Administration, Business, Marketing, or related field preferred
  • 1–3 years of experience in healthcare outreach, provider relations, or business development preferred
  • Experience in skilled nursing, post-acute care, hospice, or home health strongly preferred
  • Strong interpersonal and relationship-building skills with the ability to engage healthcare professionals
  • Excellent communication, organization, and follow-up skills
  • Self-motivated with the ability to manage time and priorities independently
  • Proficiency in Microsoft Office; experience with CRM systems preferred
