Key Responsibilities
- Conduct comprehensive in-home client assessments.
- Complete and submit required assessment documentation and care plans.
- Evaluate clients' physical condition, safety, and care needs.
- Ensure compliance with Indiana Medicaid Waiver and state regulations.
- Communicate findings and recommendations to agency staff.
- Maintain accurate and timely clinical documentation.
- Active Registered Nurse (RN) license in the State of Indiana.
- Minimum 1 year nursing experience preferred.
- Experience with home care, case management, or Medicaid waiver services is a plus.
- Strong assessment and documentation skills.
- Flexible schedule.
- Ability to accept assignments based on availability.
