- Chronic Care Management (CCM)
- Provides direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
- Leverages the use of the Electronic Health Record ("EHR") to manage and improve patient care, workflows and clinical compliance.
- Provides patient advocacy in the health care system to accomplish cost-effective care by coordination of health care services.
- Promotes effective communication with patients and encourages the role of the patient as a partner in health care.
- Must be board-certified in Internal or Family Medicine
- Completed residency in their specialty
- Active and in good standing Medical Licensure in the state of California, and Board Certified
- An active and good-standing NPI number
- An active and good-standing DEA number from the US Drug Enforcement
