Summary
Seeking a Family Nurse Practitioner to provide timely clinical care to patients presenting without scheduled appointments. The FNP will evaluate presenting concerns, provide appropriate treatment, coordinate referrals, and help patients navigate available on-site healthcare services to ensure quality clinical outcomes.
Responsibilities
- Evaluate and provide immediate care for patients with unscheduled medical needs.
- Assess presenting complaints and determine appropriate clinical treatment or referral.
- Provide health education and counseling to patients.
- Coordinate follow-up appointments and on-site referrals.
- Assist patients in accessing appropriate healthcare services.
- Provide individual and group health education.
- Collaborate with clinical and patient services staff.
- Provide clinical coverage at other affiliated sites as needed.
- Maintain high-quality, patient-centered care.
- Family Nurse Practitioner certification preferred.
- Board Certified.
- Active DEA Registration.
- HIV Care Certification.
- 2-5 years of Nurse Practitioner experience.
- Experience or training in a community health center setting preferred.
- Strong interpersonal and problem-solving skills.
- Computer proficiency required.
- Bilingual skills in Spanish, French, or Haitian Creole are a plus.
Salary : $140,000 - $148,000
