Responsibilities:
- Foster Meaningful Connections: Build and nurture relationships with healthcare professionals, providers, community-based organizations, senior housing communities, post-acute facilities, and senior centers to encourage referrals to the myPlace Health PACE program across South Los Angeles.
- Partner for Success: Collaborate closely with Enrollment Specialists to ensure a smooth, respectful, and welcoming enrollment process for prospective participants and their families.
- Engage the Community: Organize and host community events and deliver compelling presentations to share the life-changing benefits of the PACE program with prospective participants, families, caregivers, and community partners.
- Drive Predictable Growth: Support monthly outreach activity and referral goals by generating qualified leads and strengthening referral pathways throughout South Los Angeles.
- Keep Us Connected: Maintain accurate, timely records of outreach activities, partner engagement, and lead status in CRM software to ensure a seamless flow of information.
- Champion Compliance: Follow DHCS and CMS regulations and myPlace Health policies with care and precision to uphold the integrity of our marketing and outreach practices.
- Plan for Growth: Develop and implement thoughtful quarterly marketing plans designed to generate qualified enrollments and expand awareness of the PACE program across South Los Angeles.
- Be Present in the Community: Attend community and healthcare events to promote the PACE program, strengthen relationships, and grow our referral pipeline.
- Innovate and Improve: Continuously explore new opportunities to refine and elevate referral processes, outreach strategies, and community engagement activities.
- Embrace Flexibility: Be open to supporting occasional evening and weekend events as needed to meet community needs.
- Support Organizational Growth: Take on additional responsibilities as needed to contribute to the success and growth of myPlace Health.

Qualifications and experience:
- Proven Experience: Demonstrated success in healthcare or enrollment sales, with a strong track record of generating business-to-business or business-to-customer referrals.
- Knowledge of Eligibility Processes: Experience working with state and county eligibility workers to support enrollment efforts.
- Tech-Savvy: Familiarity with CRM software is a plus, though not required—we’re happy to provide training!
- Passion for Service: Previous experience working with frail and aging populations, showcasing your compassion and commitment to enhancing their quality of life.
- Self-Starter Attitude: A highly motivated individual who thrives with minimal supervision and takes initiative to achieve goals.
- Educational Background: A bachelor’s degree in healthcare, Business Administration, Marketing, or a related field is preferred but not mandatory—equivalent experience is warmly welcomed.
- Driving Requirements: A valid California driver’s license, reliable personal transportation, a good driving record, and auto insurance are required to support travel within the community.
- Language Skills: Bilingual proficiency in a language commonly spoken in Los Angeles (such as Spanish, Korean, Cantonese, Mandarin, or Vietnamese) is highly valued but not required.
- Community-Focused Role: This position is primarily field-based, working in vibrant community settings such as government offices, resource centers, senior centers, tiny home communities, and other areas ideal for generating referrals.
- Hybrid Role: This position offers a hybrid schedule, balancing time between in-office responsibilities and field-based outreach in the Greater Los Angeles market.
