Role Description

This is a full-time on-site role for a Case Management Specialist, based in Augusta GA. The Case Management Specialist will provide comprehensive case management services to individuals and families, coordinate supportive services, and assist clients in addressing housing and social service needs. Key responsibilities include conducting assessments, developing service plans, maintaining detailed case records, and collaborating with local agencies to ensure effective service delivery. The role also requires continuous follow-up with clients to monitor progress and provide guidance for achieving stability and self-sufficiency.

Qualifications

• Strong expertise in Case Management and the delivery of Case Management Services
• Background in Social Work and experience working with or navigating Social Services
• Excellent Communication skills, both verbal and written, for engaging with clients and partner organizations
• Ability to manage multiple cases while maintaining accurate and organized documentation
• Proficiency in navigating federal or state programs is a plus
• Bachelor’s degree in Social Work, Human Services, or a related field
• Compassionate and client-focused approach to service delivery
