What are the responsibilities and job description for the MICA Case Manager position at Project Hospitality?
SUMMARY:
To assist clients in acquiring permanent housing and provide case management.
RESPONSIBILITIES:
- Conduct apartment searches and escorts clients to lease negotiations with landlords.
- Make housing-related referrals such as furniture, housewares, maintenance, landlord/tenant, utilities, etc, and assist in apartment set-up.
- Design and implement individual client service plans according to specific timeframes outlined by the supervisor.
- Maintain current and accurate client charts and documentation as per standards outlined by the supervisor.
- Visit clients on a timely basis as per the program standard and make follow-up calls. Accompany clients with medical appointments and housing-related appointments.
- Schedule all field visits with the supervisor and provide documentation of visits within 2 business days.
- Attend ongoing case conferences and training.
- Refers client to internal and external service providers as needed for example legal, substance abuse, etc.
- Conduct housing workshops for housing readiness.
- Attend interdisciplinary meetings and weekly supervision.
- Work with supervisor in a cooperative and collaborative manner, including following directions and performing assigned duties.
Requirements:
- Bachelors degree and/or 5 years of social service experience.
- Some experience in case management and with the mentally ill chemically addicted population is preferred.
- Housing/real estate knowledge preferred.
- Must have a clean and valid driver’s license.
- The ability to establish workload priorities and balance diverse duties is needed.
- Must have excellent communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.
