What are the responsibilities and job description for the Registered Nurse position at PROJECT HOSPITALITY INC?
Job Duties/Responsibilities:
1. Conduct onsite and on-call health services in 24 hour, 7days week residential facility. Ensure coverage
nurses provide health services in accordance with regulatory and agency requirements.
2. Health Monitoring pre- admission and ongoing withdrawal symptom and health screenings. Review all
intake health and medical documents. Complete CIWA and COWS tools. Consult with PD and/or MD for intake result. Communicate client
information with Intake staff. 3. Conduct admission clinical health screening and/or health evaluations
and follow-up withdrawal symptom screening; consult with PD/MD for admission decision.
4. Health Coordination- Ensures the provision of education, risk reduction, counseling and referral
services to all residents regarding HIV and AIDS (including pre and post-exposure prophylaxis),
tuberculosis, viral hepatitis, sexually transmitted diseases and other communicable diseases.
5. Communicate to MD all client medical concerns and health referral needs. Document all clients'
services in the electronic health records in a timely manner. Review all LPN client services
documentation to ensure compliance and clinical appropriateness. Clinical documentation in electronic
health records in compliance with Medicaid billing, OASAS/DOHMH and other regulatory requirements.
6. Supervise daily task of LPN including daily medication supervision and client health monitoring, and
other health interventions.
7. Conduct monitoring of health indicators identified through a physical examination and/medical or
other health screening. The health monitoring will involve:
a. Individual Sessions to determine client medical baseline and medical needs
b. Create health treatment plans in coordination with medical and clinical team
c. Conduct individual health screenings for clients to determine clients health issues and daily
living needs and to create a baseline for monitoring health conditions including, CBC panel blood
testing, alcohol and drug screening, tobacco use screening and smoke cessation
intervention/education, blood pressure, cardiovascular disease (CVD) risk, respiratory
conditions, weight and obesity (body mass Index, BMI; waist circumference, WC); blood
pressure; thyroid, dietary intake; activity level and exercise and other health issues. Ensure
health screening tools are maintained in a safe and working cabinet. Order materials as need for
health screening.
8. Supervise all aspects of client medication prescriptions and medication management. Create and
manage medication logs for medications received from pharmacy, ensure proper medication storage
and safety, conduct injections, update and maintain client injection episode in medication log book.
Ensure all staff are trained in appropriate medication monitoring and storage.
9. Manage all medical testing services, including ensuring client appointments, testing equipment and
materials, pre and post testing lags, and reporting to regulatory and/or agency sources as required.
10. Conduct smoke cessation sessions and monitor client use of NRT supports.
11. Conduct small group health sessions and focus on 1) educating clients on medication and treatment
compliance strategies, 2) educating clients on disease/health disorders, 3) providing smoke/tobacco
cessation interventions/education.
12. Conducting intermittent health screenings to measure changes in health status/indicators.
13. Conduct nutrition, diet, and daily activity education and Interventions, alcohol and drug education
and other health monitoring activities. The nurse will track changes in health Indicators and record in
client electronic health records.
Knowledge and Skills:
Working knowledge and experience working with substance using and co-occurring disorders clients.
Experience conducting health screenings in mental health, health and substance abuse areas.
Experience conducting group health education sessions. Familiarity with trauma informed
environments. Experience with clinical documentation, use of electronic health records systems and
Medicaid billing procedures,
Valid RN Licensed by NYS Department of Education. Licensed Registered Nurse, at least three year post
licensure providing nursing care in hospital and/or clinic setting. Physical ability to perform the essential
job functions is needed. In addition, the Licensed Registered Nurse will perform any dutles as required
to meet the needs of the program and the clients that we serve. The Agency reserves the right to revlew

Salary : $87,000 - $139,000
