Project FoodBox is transforming healthcare by delivering medically tailored groceries and nutrition support to people managing chronic health conditions.
We're looking for compassionate, outgoing Community Health Workers who enjoy connecting with people and making a meaningful impact. In this role, you'll represent Project FoodBox at community events, educate eligible individuals about our medically tailored meal program, and help them enroll in services that can improve their health.

What You'll Do
As a Community Health Worker, your primary responsibility is helping eligible community members enroll in our medically tailored meal program.
Responsibilities include:
- Engage with community members at health fairs, resource events, and outreach activities.
- Explain the benefits of medically tailored meals and how they support chronic conditions such as diabetes and high blood pressure.
- Screen individuals for program eligibility.
- Complete enrollment applications accurately using company systems.
- Collect contact information for individuals interested in learning more.
- Build trust through compassionate, culturally responsive interactions.
- Answer questions about the program and next steps.
- Connect individuals with additional community resources when appropriate.
- Assist with event setup, breakdown, and on-site logistics.
- Maintain accurate documentation while protecting confidential health information.
- Follow HIPAA, CMS, and company compliance requirements.
- Work collaboratively with outreach coordinators and other team members to ensure successful events.
What We're Looking For
- Community Health Worker (CHW) certification required.
- Previous experience in community outreach, healthcare, customer service, enrollment, event staffing, or a related field is preferred but not required.
- Strong communication and interpersonal skills.
- Comfortable starting conversations and engaging with diverse populations.
- Passion for helping others and improving community health.
- Strong attention to detail and ability to accurately complete documentation.
- Ability to handle confidential information professionally.
- Reliable transportation and willingness to travel to community events as needed.
- Ability to stand for extended periods and lift event materials when necessary.
- Bilingual (Spanish/English or another language) is highly preferred.
