Today Project FIND operates three supportive housing residences that are home to about 600 people and four senior centers with over 3,000 members. Please visit our website at: www.projectfind.org.Job Summary:Case Management services help older persons with functional impairments gain access toappropriate services, benefits and entitlements needed to age safely at home andtomaintaintheir quality of life.Case Managersmust work todevelop trusting relationships with clients and their caregivers and engagethem in a collaborative process of problem solving.ACase Managerwillbe responsibleformanaging a caseload of65 clients andwillprovidesupportthat isconsistent with the goals of being professional,timely, culturallyawareand sensitive,whileassistingclientstoachievemaximum potential as defined by their abilities.Responsibilities include:Screen new potential case management clients through the intake process.Interview clients with problems, such as personal and family adjustments, finances, physical and mental impairment, todeterminenature and degree of problemProvideaccurateandappropriate informationon services,benefitsand entitlements.Assistthe person on intake as an intake back-up when scheduled.Conduct in-home client assessments todetermineeligibility for service(s).Work with existing client informalsupportstooffer assistanceand encourage ongoing efforts.Develop care plans consistent with client needs that reflect culturally sensitive optionsthat make efficient use of community resources.Conduct in-home reassessments for clients todetermineeligibility for services.Arrange for provision of services, advocate and intervene with other agencies on behalf of the clients.Compile andmaintaincompleteandaccuratecasefiles.Developandmaintainpartnershipswith otherprovidersand resources.Comply withNYC Aging training requirements toensure the completion of 16hours of training relevant to case management practice/program administration.Collaborate with other departments/programs as needed (assistother staff members in helping a member/client).Assistand support clients with the need for Supplemental Services Funds (identifyneeds and collaborate inpurchasingand delivering items).Develop an understanding of agencymission, history,philosophyand programs.Participate in outreach efforts when necessary.Participate instaffdevelopment activities asrequired,becomefamiliar withProject FINDprograms, andregularly attendstaff meetings.Participate in and completeannualevaluationprocess.Respond well to supervision.Work independently whilemaintainingappropriate communicationwithsupervisor.Performappropriate jobtasks as needed.Observe good attendance and punctualityonthe job.Qualifications:MSW degree(preferred)or relatedmasterslevel degree(e.g.social services, public administration,nursingor public health).OrBachelorsdegree and at least 2 years of experience working in the field of aging.OrAssociatesdegreewith four years or more of casework experience ina community social service or social action program.Mustdemonstrateculturalcompetence,beknowledgeableandsensitive to the diverseneeds of the communities served, including clients with visual and/or hearing impairments.Must be able to multi-task.BilingualEnglish/Chinesespeaker preferred and mustdemonstrateverbal and written competencies.Ability to gather and summarize data for reports, find solutions tochallengesrelated to the clients.Ability to apply principles of logical thinking to a wide range of intellectual and practical problems.Mustbe able toprioritize work.Demonstrate excellent interpersonal and communicationandstrongcustomer serviceskills.Must exerciseexcellent follow-through on assignments.Schedule:Monday - Friday 9:00AM - 5:00PMProject FIND participates in E-Verify, which confirms employment authorization through an electronic database maintained by the Social Security Administration and Department of Homeland Security.
