What will you be doing in this role?
- Provide direct patient support including vitals, intake, triage, and clinical preparation
- Administer medications and immunizations within LPN scope of practice
- Support chronic disease management plans under provider direction
- Assist with clinical procedures, wound care, and diagnostic testing
- Document all care accurately in the clinic’s EMR (EPIC or designated system)
- Educate patients and families on medication use, follow-up care, and preventive health
- Participate in infection control, quality improvement, and safety processes
- Support occupational health services as applicable (injury triage support, screenings, wellness assessments)
- Collaborate with the multidisciplinary clinical team to provide coordinated, patient-centered care
Our ideal candidate will have:
- Active, unrestricted LPN license in state of practice
- Does not currently have any professional disciplinary proceeding of any state or federal authorities or any disciplinary action of any medical facility in any jurisdiction
- Has never been excluded, suspended, or otherwise sanctioned by Medicare, Medicaid, or any other third-party payor, nor listed on any state or government list of excluded individuals, parties, or specially designated nationals list as debarred, excluded, or otherwise ineligible for participation in government healthcare programs.
- CPR/AED/BLS certification or the ability to obtain it prior to start of employment; maintain and provide proof of current certification(s)
- Experience in primary care, after-hours care, community health, or rural health preferred
- Proficiency in medication administration, injections, immunizations, and treatment support
- Strong commitment to culturally respectful and trauma-informed care
- Ability to work independently and collaboratively in a clinical team
- Excellent communication skills and ability to maintain strict confidentiality
- Basic computer proficiency, including Microsoft Office and EMR systems
