The purpose of the Central Intake Coordinator is to establish an effective system to gather information and communicate with the referral source before and upon admission to assure proper placement of the resident. Also, to identify from among the residents seeking admission those for which the facility can provide adequate care. This is a remote position. This position will be responsible to cover referrals one weekend a month.
Duties:
- Gather sufficient pre-admission data to determine resident level of care and the ability of the facility to properly care for the resident.
- Ability to collect accurate data for all payer sources.
- Ability to interact with discharge planners in an effective, efficient manner. Ask appropriate and adequate resident information. Ability to analyze the information to determine if the facility can care for the resident adequately.
- Perform all duties as assigned in an effective manner.
- Follow admission and transfer policies and procedures.
- Keep accurate records of residents’ information; keep all resident’s information confidential.
- Work cooperatively with all facility department supervisors and administrative staff.
- Accept assigned duties in a cooperative manner.
- Ability to understand the facility computer system and the software used in the department.
- Come to work in neat, clean attire and consistently present an appropriate professional appearance.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Consistently work cooperatively with resident, resident’s representative, facility staff, physicians, consultant and ancillary service providers.
