Job Overview
We are seeking a dedicated and experienced Designated Coordinator to oversee care operations within our facility. The ideal candidate will possess extensive knowledge of 245D regulations and group home administration. This role involves managing care teams, ensuring compliance with health policies, and coordinating person-centered services to promote well-being and quality of life for our individuals.
Responsibilities
- Lead and supervise teams, including direct support staff and house managers, to deliver exceptional person-centered care.
- Develop, implement, and monitor individualized care plans that address individuals' specific needs.
- Ensure compliance with all applicable 245D regulations, health policies, and safety standards.
- Coordinate with healthcare professionals, families, and community services to support individuals’ health and social needs.
- Maintain accurate documentation related to individuals' care, medication administration, and regulatory reporting.
- Foster a positive environment that promotes leadership development among staff members while encouraging teamwork and professional growth.
Experience
- Extensive experience in group home administration with a strong understanding of 245D regulations.
- Proven supervisory experience managing multidisciplinary teams within healthcare or residential care settings.
- Demonstrated strong knowledge and a comprehensive understanding of developmental disabilities and mental health conditions, applying this expertise effectively to support individuals with diverse needs.
- Familiarity with healthcare financial management, budgeting, and resource planning.
- Proficiency in Microsoft Office applications for documentation and reporting purposes.
- Background working with individuals with developmental disabilities or in disability services.
- Bachelor's degree in a human services-related field (such as social work, psychology, sociology, special education, or rehabilitation counseling) and have at least one year of full-time experience providing direct care services to people with disabilities or people age 65 and older.
- Valid Drivers License
