Job Summary
The Hospice Aide provides personal care and supportive services to hospice patients under the direction of a Registered Nurse, following the physician’s plan of care. This role supports patient comfort, dignity, and safety in the home setting.
Qualifications

Certified Home Health Aide (CHHA) – Oklahoma

Hospice or direct patient care experience preferred

Valid driver’s license, reliable vehicle, and auto insurance

Ability to travel within the service area

Strong communication skills

Comfortable working in diverse home environments
Key Responsibilities

Provide personal care per the plan of care
-
Assist with mobility, transfers, and ambulation

Support patient comfort and hygiene
-
Assist with meals and elimination needs

Monitor and report changes in patient condition

Record vital signs as ordered

Perform light housekeeping related to patient care
Documentation

Complete accurate and timely visit documentation
-
Documentation completed within 48 hours
