What You'll Do

As a Hospice Aide, you will:

• Provide compassionate personal care to hospice patients under the direction of the Registered Nurse and according to the individualized Plan of Care.
• Assist patients with bathing, dressing, grooming, toileting, and other activities of daily living while preserving dignity and independence.
• Safely assist patients with transfers, ambulation, and mobility according to agency policies.
• Observe, report, and document changes in patient condition to ensure continuity and quality of care.
• Maintain accurate and timely documentation of services provided.
• Practice infection prevention and maintain a clean, safe, and comfortable environment for patients.
• Assist patients with nutrition, hydration, and meal preparation as appropriate.
• Communicate respectfully and compassionately with patients, families, and members of the interdisciplinary care team.
• Protect patient privacy, confidentiality, and personal property at all times.
• Perform assigned treatments and procedures in accordance with agency policies.
• Support the mission, values, and philosophy of Choice Hospice while contributing to exceptional patient care.

Qualifications

Required:

• Current Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification
• Current Long-Term Care (LTC) certification (if required by state)
• Current CPR Certification
• Valid Driver's License
• Current Auto Insurance
• Reliable transportation
• Ability to travel to patient homes within the assigned territory

Preferred:

• HomeCare HomeBase experience
• Previous hospice or home health experience
