Registered Nurse (RN) - Primary Care/Care Management

Progressive Health of Delaware is looking for a Registered Nurse to join our Primary Care team at our Newark office. The schedule is dayshift Monday - Friday, with an option for flexible scheduling. This position would primarily be in-office, with opportunities for some remote/hybrid work. No holidays or on call!

We are a primary care medical practice, that is dedicated to lifestyle medicine and wellness. We are experts in how to help people be as healthy as possible! The RN will support all aspects of patient care, with a focus on patient triage, high-risk patient care management support, patient education and other clinical support for the physicians and advanced practice clinicians.

• Experience in an outpatient/ambulatory setting is a plus
• Must have active and unrestricted licensure as a Registered Nurse (RN) meeting all state certification and licensing requirements in the state of Delaware
• Minimum of two (2) years of nursing experience related to chronic care management and chronic health conditions
• An understanding of Lifestyle Medicine and passion for this approach to health preferred
• Knowledge of serious medical conditions (such as COPD, CHF, cancer, dementia, diabetes, ESRD, etc.) and their progression
• Excellent phone/video communication skills
• Experience in Motivational Interviewing preferred
