Job Requirements/Responsibilities
- Current, unencumbered Indiana license.
- Preferred two years of experience in acute or sub-acute care setting.
- Must meet credentialing requirements for Medicare, Medicaid, and Managed Care Organizations.
- Evaluation and management of clients with multiple medically complex health conditions.
- Ability to function autonomously in a fast-paced environment.
- Superior Communication skills.
- Excellent organizational skills and effective time management capabilities.
- Collaborates with facility personnel to ensure multi-disciplinary approach.
- Desire to provide ongoing education to LPNs/RNs on a routine basis.
- Maintains positive and professional attitude and demeanor.
