Are you a CNA, QMA, Medical Assistant, or healthcare professional who enjoys working with seniors?
Proactive Clinical Partners is looking for a Clinical Navigator to help connect residents in skilled nursing and assisted living facilities with our physicians and nurse practitioners through telehealth.
You’ll travel to assigned facilities, set up telehealth visits using a company-provided iPad, and help residents connect with their healthcare provider.
No hands-on clinical care or diagnosing is required. Your role is to help make sure residents can be seen by their provider when an onsite provider is unavailable.
What You'll Do
- Travel to assigned nursing and assisted living facilities.
- Help residents connect with physicians and nurse practitioners through telehealth.
- Set up and operate a company-provided iPad and telehealth equipment.
- Coordinate with facility nurses and staff during visits.
- Help ensure visits are private and HIPAA compliant.
- Keep track of completed visits and mileage.
- Communicate any technology or scheduling issues to the clinical team.
- Build positive relationships with residents and facility staff.
What We're Looking For
Required:
- High school diploma or equivalent.
- Valid Indiana driver's license.
- Reliable vehicle and current auto insurance.
- Comfortable driving between facilities.
- Comfortable using an iPad and video-calling technology.
- Strong communication and organizational skills.
- Comfortable working independently with elderly residents.
- Ability to pass a background check and required facility screenings.
Preferred:
- CNA, QMA, or Medical Assistant certification.
- Experience in skilled nursing, assisted living, or long-term care.
- Experience working with elderly or geriatric patients.
- Previous telehealth or EHR experience.
Schedule & Pay
- Part-time
- Approximately 4–5 hours per day
- Days and locations may vary based on coverage needs.
- $18–$20 per hour, based on experience and certification.
- Mileage reimbursement for travel between facilities.
- Company-provided iPad and telehealth equipment.
