What are the responsibilities and job description for the Chronic Care Management (CCM) Coordinator, Nurse position at Proactive Clinical Partners?
Proactive Clinical Partners is a physician-owned geriatric medical practice caring for residents in approximately 28 skilled nursing, assisted living, and senior living communities across Indiana. Our team of 20-plus physicians and nurse practitioners focuses on a single population, older adults with complex, ongoing health needs.
We are building our Chronic Care Management program in-house, and we are looking for a nurse who wants to help shape it from the ground up. This is an opportunity to stay close to patient care while working a predictable, hybrid schedule.
Requirements
- Active, unrestricted Indiana LPN license (RN welcome)
- 2 years of clinical experience, with geriatric, long-term care, or care coordination experience preferred
- Chronic Care Management, Transitional Care Management, or case management experience is a strong plus
- Comfortable with electronic health records and phone-based patient work
- Self-directed and reliable in a hybrid setting, with a secure, private workspace for remote days
- Organized, detail-oriented, and committed to accurate documentation and HIPAA compliance

Pay: $25.00 - $30.00 per hour
Experience:
- Long term care or care coordination: 2 years (Preferred)
License/Certification:
- unrestricted Indiana LPN license (Required)
