Overview
I am seeking a dedicated and compassionate Private Duty Nurse to provide personalized care to a patient in her home. You will work closely with the family to develop care plans that meet their unique needs, while also adhering to established healthcare standards. The patient has ForwardHealth for payment (hours and payment are submitted through Medicaid). I'm looking for 2 days per week (7:30a - 5:30p), there may be opportunities for more hours. Ventilator certification is a requirement
Responsibilities
- Deliver high-quality nursing care to a patient (8F) in home and school settings, focusing on individual needs and preferences.
- Monitor patient health by performing assessments and documenting vital signs and medical history.
- Administer medications and treatments as prescribed by physicians, ensuring adherence to safety protocols.
- Collaborate with other healthcare professionals to coordinate comprehensive care plans, including critical care transport when necessary.
- Maintain accurate records of patient care activities in compliance with ICD-10 standards.
- Uphold the highest standards of nursing practice while fostering a supportive environment for the patient.
Experience
- A valid nursing license is required, with experience in internal medicine or long-term care preferred.
- Familiarity with basic life support techniques is essential.
- Previous experience in a teaching hospital setting or critical care transport is advantageous.
- Knowledge of hospice palliative medicine practices is a plus.
- Strong communication skills are necessary for effective interaction with patients and families regarding their aliment and healthcare needs.
