Essential Functions

- Serves as the primary clinical liaison between referral sources, the home infusion pharmacy, and the interdisciplinary care team to facilitate timely initiation of home infusion therapy.
- Performs clinical screening and assessment of referred patients to evaluate appropriateness of home infusion therapy based on diagnosis, prescribed treatment, patient readiness, and home environment considerations.
- Collaborates with intake coordinators, pharmacists, nursing staff, and providers to coordinate therapy initiation, nursing services, medication delivery, and start of care timing.
- Educates patients and caregivers on prescribed infusion therapies, vascular access devices, equipment use, and safety considerations to support successful therapy in the home setting.
- Communicates clinical information and recommendations to referring providers, case managers, and discharge planners to ensure continuity of care and appropriate transitions from inpatient or outpatient settings.
- Identifies and addresses clinical barriers to home infusion therapy initiation, escalating complex cases or concerns to leadership or medical providers as appropriate.
- Supports business development efforts by building and maintaining relationships with hospitals, clinics, physicians, and referral partners, serving as a clinical resource to promote home infusion services.
- Participates in referral source education and outreach activities to increase awareness of home infusion capabilities, clinical appropriateness, and service offerings.
- Ensures accurate, timely documentation of clinical assessments, patient education, and coordination activities in accordance with regulatory, accreditation, and payer requirements.
- Participates in quality improvement and performance initiatives related to home infusion clinical services, contributing clinical insight to improve patient outcomes and operational efficiency.
- Performs other duties as assigned.
- This is a non-management job that will report to a supervisor, manager, director or executive.
- Education - Associates degree in Nursing. BSN preferred
- Experience - One (1) year of nursing experience.

- Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
- A valid driver’s license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance. Driving is required for this position using the employee’s personally owned vehicle. Employee
