Social Worker Designee

Monday-Friday 8am-5pm

  • Position Overview:**

The Social Worker Designee plays a crucial role in the nursing home setting by providing emotional support, counseling, and resources for residents and their families. This role involves assessing the social and emotional needs of residents, facilitating communication between staff and families, and advocating for the well-being of each resident.

  • Key Responsibilities:**
  • **Assessment and Care Planning:**
  • Conduct comprehensive social assessments of residents to identify needs, strengths, and potential issues.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that address social, emotional, and psychological needs.
  • **Counseling and Support:**
  • Provide direct emotional support and counseling to residents and their families, addressing concerns related to adjustment, loss, and mental health.
  • Facilitate support groups and educational sessions for residents and families.
  • **Advocacy:**
  • Advocate for resident rights and needs, ensuring their voices are heard within the facility.
  • Assist residents in navigating health care services and accessing community resources as necessary.
  • **Family Engagement:**
  • Develop and maintain effective communication with family members, providing updates on resident progress and care.
  • Help mediate conflicts between family members and the staff to ensure a supportive environment.
  • **Crisis Intervention:**
  • Identify potential crises and provide immediate intervention when needed, including referrals to mental health professionals if required.
  • **Documentation and Compliance:**
  • Maintain accurate and timely documentation of resident assessments, care plans, and progress notes in compliance with regulatory standards.
  • Ensure adherence to facility policies and state regulations regarding social services.
  • **Training and Supervision:**
  • Provide training and guidance to staff on social work practices, resident rights, and best practices for supporting residents with psychosocial challenges.
  • Qualifications:**
  • A Bachelor’s degree in Social Work (BSW) or related field; a Master’s degree (MSW) is preferred.
  • Current licensure or certification in social work as required by state regulations.
  • Experience in a healthcare or long-term care environment is highly desirable.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work collaboratively within an interdisciplinary team.
