What are the responsibilities and job description for the Admission Advocate (Central Intake) position at Princeton Transitional Care?
Princeton Transitional Care Assisted Living is a partnership between Signature HealthCARE and Ballad Health bringing a new level of health care services to the Tri-Cities area. The new Princeton Transitional Care will provide a new home for skilled-nursing care and will add 60 assisted living apartments, 47 short-term skilled-nursing and rehab private suites. The state of the art community will feature a streetscape with a chapel and theatre, library and media center, Blue Ridge market, coffee bar, ice cream bistro, billiard parlor, outdoor walking paths, and on-site physician office. The new one-story, 73,000-square-foot facility will be located on Princeton Road on the site of the former North Side Hospital.

As our Admissions Advocate (Central Intake), you will make a difference by serving as an advocate to help prepare new residents and families for their transition to a Signature HealthCARE facility. Your role will consist of ensuring potential admissions are prepared for a smooth transition by communicating admissions and transitional care needs with the appropriate stakeholders involved in care. To be successful in this role, you will need to understand census, quality mix, and bed management needs of the building. You will also need to maintain a working knowledge of Federal and State regulations, referral processes and reimbursement (Managed Care, Medicare, and Medicaid). This role will also require that you spend time in hospitals meeting with potential residents and families. Local travel with your personal vehicle is required.

Requirements for Consideration include:
- Minimum of one (1) year experience in long-term care or related industry.
- Must understand long-term care business, including census and quality mix goals and referral expectations.
- Demonstrated intermediate to advanced skills in Microsoft Office Suite.
- Must have highest level of professionalism with the ability to maintain confidentiality.
