Prime Home Developmental Disability Services (PHDDS) is a certified provider through Nebraska DHHS, offering a wide range of residential and day services for individuals with developmental and intellectual disabilities. Our mission is to provide a full circle of care through individualized programs that empower people to live fulfilling, independent lives.
What You’ll Do
- Work Monday–Friday during daytime hours
- Manage a caseload and coordinate care
- Communicate with employees, guardians, Shared Living Providers, and interdisciplinary teams
- Review staff documentation and ensure proper staffing ratios
- Hire, train, supervise, and develop Direct Support Professionals (DSPs)
- Support clients in daily living skills: nutrition, hygiene, exercise, communication, and socialization
- Intervene in crisis situations and refer to appropriate care
- Use behavioral de-escalation techniques (paid MANDT/SCM training provided)
- Implement treatment plans and contribute to program development
