Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes to healthcare providers promptly
- Support medication administration following prescribed guidelines while maintaining accurate medical documentation
- Provide specialized care for conditions such as Alzheimer's disease, dementia, and memory care needs
- Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient interactions and care activities accurately
- Assist with catheter care, wound care, Hoyer lift transfers, and other medical procedures as trained
- Support patients with meal preparation, feeding assistance, laundry, cleaning, and maintaining a safe living environment
- Follow HIPAA regulations to ensure patient confidentiality at all times
- Collaborate with nursing staff and other caregivers to develop and implement personalized care plans
- Engage in hospice or palliative care activities when applicable to provide comfort and emotional support
Experience
- Prior experience working in assisted living facilities or nursing homes is highly preferred
- Demonstrated ability to work effectively with people with developmental disabilities or disabilities of any kind
- Experience providing dementia or Alzheimer's care in home or long-term care settings is advantageous
- Familiarity with medical terminology and medical documentation practices is beneficial
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice care or catheter care are a plus
- Proven ability to operate EHR/EMR systems efficiently and accurately record patient data
- Long-term care experience or home & community care background preferred for success in this role
