School-Based Navigator Summary
Under general supervision, provides coordination for students navigating through School Based Health care, identifies and reduces barriers to care, and performs a variety of outreach and public relations throughout the school community.

• Establishes rapport with school nurses.
• Communicates in an understandable manner and monitors students’ understanding.
• Triages phone calls and walk-ins as needed.
• Assists patients in answering routine questions, resolving minor difficulties and giving directions to patients as authorized by the Provider.
• Communicates student care plan with parent/guardian.
• Uses knowledge of student’s cultural, ethnic, religious and social system to develop rapport.
• Identifies and assists families in reducing barriers to care and adhering to plan.
• Schedule appointments and coordinate transportation to and from health center.
• Works to ensure that school – based consents and registrations are complete.
• Develop an efficient mapping process to move the most patients in the least disruptive amount of time.
• Arranges follow-up.
• Coordinate immunization clinics for each school.
• Coordinate vision and dental screenings for each school.
• Promotes positive interpersonal (customer) relationships with fellow employees, providers, patients and school staff. Treats these individuals with courtesy, dignity, empathy and respect; consistently displays courteous and respectful verbal and non-verbal communications.

• Ensure confidentiality of patient information by creating and maintaining a secure and trusting environment by not sharing information learned on the job, except when necessary, in the performance of the job responsibilities or to improve a patient’s care.
• Has regular and predictable attendance.
• Maintains pertinent databases.
• Performs all other duties and tasks as assigned.
Clinical:
• As directed by the Provider, assists with routine office examinations and procedures that may include administering injections, medications, vaccinations and blood draws. Documents all procedures in the patient’s chart.
• Documents all patient interactions, tracks and attaches laboratory and diagnostic test results to patient charts; communicates results and orders to patients as directed by the Provider.
• Contacts pharmacy to order new and refill prescriptions and follows up with patients as needed.
Operations:
• Prepares, cleans and sterilizes equipment and exam rooms as established by standard safety and clinical protocols.
• Properly handles the disposal of infectious/hazardous waste, according to OSHA guidelines.
• Inventories and requests supplies.
• Responsible for performing health and team based care related services within the guidelines of the state of Ohio Board of Nursing.
• Support a shared goal model to achieve high-quality care that is patient and family centered.
• Participates in scheduled pre-visit planning/huddle process.
• Facilitates coordination of care following hospital, ER utilization or follow up from specialty providers.
• May participate in team meetings and quality improvement activities as necessary.
• Performs all other duties and tasks as assigned.

Education/Experience
Graduation from an accredited school of nursing.
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills
To perform this job successfully, an individual should have the ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, text paging, Internet, and Intranet.
Certificates, Licenses, Registrations
LPN certification. Must hold a current, valid license to practice practical nursing in the state of Ohio.
Other Applicable Requirements
Knowledge: Basic care coordination (navigating & mapping), knowledge of community resources
Skills: Establish and maintain rapport with school community. Uses computers, including Microsoft Outlook, Word, PowerPoint, Excel, i2iTracks, scheduling software (NextGen) as well as perform Web searches for clients, BLS. Abilities: Bilingual English/Spanish or English/other languages preferred.
